There are two versions of the CRAFFT 2.1: a Clinician Interview and a Self-administered Questionnaire. Research has shown that adolescents report greater comfort and likelihood of honesty with self-administered questionnaires (electronic versions or on paper) compared to face-to-face interviews. The self-administered version is also more time-efficient to administer than an interview. Therefore, we recommend the use of the Self-administered Questionnaire whenever possible, under conditions that protect patient privacy and confidentiality. The responses can then be used by the healthcare provider during the appointment to facilitate brief counseling.
The CRAFFT+N versions contain extra questions that are related to tobacco and nicotine use. These are available as a Clinician Interview and a Self-administered Questionnaire.
To ensure confidentiality, some offices set aside a private corner of the waiting room for the patients to complete the screen pre-visit. Other offices find it more practical to have patients complete the screen in the exam room prior to their appointment with their clinician. The most important principle is to ensure that parents cannot see their children’s responses, so that adolescents feel comfortable reporting honestly.
The CRAFFT 2.1 screen has been translated into several languages. These translations have been completed using rigorous best practices, with translation, back translation, and reconciliation of identified discrepancies. Funding for these translations was provided by the Massachusetts Department of Public Health, Bureau of Substance Abuse Services and the Massachusetts Behavioral Health Partnership.
The CRAFFT 2.1 Screening Interview is intended for use by trained clinicians as a brief interview with an adolescent patient. The CRAFFT 2.1+N contains additional questions about tobacco and nicotine use.
The CRAFFT 2.1 Self-administered Questionnaire is intended to be completed in a private place by the patient either on paper or on an electronic device. The CRAFFT 2.1+N contains additional questions about tobacco and nicotine use.
Pocket Cards of the CRAFFT 2.1 Screening Interview are available for clinician use. All cards are printed on a red, laminated 4 X 5 pocket card.

Reproduce the CRAFFT
The CRAFFT questions are copyright protected by Boston Children’s Hospital; however, a goal of CeASAR is to make the CRAFFT widely available to qualified clinicians and researchers, and we welcome reproductions of this screening tool.
The following conditions must be met in order to use the CRAFFT screen in your work:
- You must submit a final draft of your reproduction (either a reprint, screen shot or electronic program) with a description of the intended context for use as an attachment to email@example.com. Please also let us know which version of the CRAFFT you have reproduced, and if you are using the Self-Administered Questionnaire or the Clinician Interview in your program.
- All CRAFFT questions must stay in their original order with no substitutions or omissions. However, you may choose to reproduce only the first page of the CRAFFT Clinician Interview.
- The following language must be printed at the bottom/end of the program:
